Mason Gross Request Form -- School to School Transfer, BA Declaration, or Re-enrollment

This form is for currently or formerly enrolled Rutgers students that are seeking to either:

• Request a School to School Transfer from another academic unit within Rutgers University
• Declare a BA while enrolled in another academic unit within Rutgers University
• Apply for Re-enrollment after having taken a leave of absence from any Rutgers academic unit or would like to return and enroll in a BFA or BM degree program at the Mason Gross School of the Arts. 

Admissions to our BFA/BM degrees  and the BA in Art declaration requires a talent assessment evaluation and an artistic application. Please note that we only accept re-enrollment applications for fall entry. 

After submitting this form, the admissions team will review your application and send you instructions on next steps in the process. This may include links to complete the Mason Gross Talent Assessment application, if required.

PLEASE NOTE:

**Students interested in declaring the BA in Dance, Music or Theater do not need to complete this form**

**Many programs only allow for fall entrance. If you are submitting for spring enrollment, the Admissions team will let review your application and verify the program you are pursuing is open for spring or if you need to reapply at later date.

**Please note as a re-enrollment applicant you may be asked for additional information if you left for academic reasons.

*** Please note that if you were artistically dismissed per the MGSA policy you are ineligible to re-enroll.
